Easy way to get services

Your First Appointment

Please arrive 15 minutes early to complete your initial paperwork and discuss any questions you may have.

If you need to cancel or reschedule, kindly notify us at least 48 hours in advance to avoid a cancellation fee.

Payment Policy

Canning Vale Dental operates a Pay on the Day policy. We accept cash, credit card, and EFTPOS. For your convenience, we also offer HICAPS to process your health fund rebate immediately after treatment.

What to Bring

To help us provide the best care, please bring:

A list of all medications, vitamins, or supplements you are currently taking.

Your Private Health Insurance Fund membership card (if applicable).

Any Department of Veterans’ Affairs card, Pensioner Concession card, Health Care card, or other documents entitling you to subsidised treatment.

General Authority Form if you receive any Government Assistance.

Any recent dental x-rays from previous treatments.
